What is the meaning of ICSR and full form ?

This is a type of report that is prepared by a physician for a patient.When a drug is administered to the patient and that drug produces side effects or negative reactions in the patient,the the attending physician or healthcare worker needs to prepare the “Individual case safety report”.This report is submitted for review to the pharmacovigilance authority as well as the FDA of that country.
